Meet Michelle, PMHNP-C, FNP-C

Michelle Fulp is a PMHNP-C (psychiatric mental health nurse practitioner) and a FNP-C (family nurse practitioner). I also have my registered nurse (RN) license with advanced training in mental health and family medicine.

I can assess, diagnose and treat patients with anxiety, depression, trauma, mood disorders, insomnia, dementia, personality disorders and other mental disorders.

This is what I do -- My role as a PMHNP is essential to changing how mental health care is viewed and to lessen stigmas associated with mental health treatment. In addition to serving as an educator, I am skilled at relationship building and collaborating with others in the health care setting.

Intake $300.00 per hour

Follow up: 45 minutes: $150.00

Follow up: 30 minutes: $130.00

Medication refill/s: 15 min visit; $70.00

Cancellation fee within 24 hours of appointment is $70.00

Credit card with back up card must be on file

Where do we meet?

Most of our consultations will be over the phone or via a video-call so you can be wherever you are most comfortable. In the event we need to meet in person, we'll make arrangements at that time.

Medical documents/forms (including FMLA, STD) will be considered only for patients established for at least two months. A fee will be applied. New patients are encouraged to arrive 15 minutes prior to their scheduled appointment time to allow enough time for the registration and check in process. Missed Appointments and late cancellations inconvenience those individuals who need access to medical care in a timely manner. A missed appointment policy enables me to better utilize available appointments for my patients in need of medical care. Cancellation of an appointment: In order to be respectful of the medical needs of other patients, please be courteous and call A Peaceful Mind promptly if you are unable to show up for an appointment. This time will be reallocated to someone who is in need of treatment. If it is necessary for you to cancel your appointment, I require that you call at least 24 hours in advance. Appointments are in high demand, and your early notification will give another person the possibility to have access to care. First late cancellation/Missed appointment: Courtesy reschedule. Second late cancellation/Missed appointment: $70.00 fee will be applied to your account . Third late Cancellation/Missed appointment: $70.00 fee will be applied to your account and may result in a discharge from the practice. To cancel your appointment: Please contact A Peaceful Mind at 480-640–4908, if you do not reach a representative, please leave a detailed voicemail including your full name, date of birth, date/time of the scheduled appointment and reason for cancellation.
